Incidents and investigations

From a phone in the field to a closed investigation with its evidence.

LMLevi Mieliv1.1

Report an incident from the field in under a minute: type, severity, photos and people involved. From there the record follows one path to closure, and every step lands in the evidence trail your auditor will follow.

The path to closure

  1. Report

Anyone on site can raise it, on their phone, at the time. Photos attach to the record rather than a group chat.

  2. Triage

Severity and type set who gets notified and how fast. Notifiable incidents are flagged as such at the point of triage, not discovered later.

  3. Investigate

Findings, contributing factors and the people involved are recorded against the incident, with the evidence attached where it was found.

  4. Act

Corrective actions land in the same one action queue as every other finding in the system, with owners and due dates.

  5. Close

Sign-off closes the incident and freezes its trail — report, investigation, actions and evidence, in order, with timestamps.
